To receive a duplicate/copy of your Virginia license please submit a $25 check or money order made payable to the Virginia Department of Education with a written request for a duplicate/copy of your Virginia license. You must include 1) your current address; and 2) your Social Security Number or license number, in the request. Mail the fee and request to:

Are you employed in Virginia in an accredited public or nonpublic school?
Please request that your employing school division submit a request for adding an endorsement, together with the following completed forms and documents, to the Virginia Department of Education, Office of Licensure.
